Here are 10 questions what we usually ask during Exit Interview with Employee, this can be on a form of questionnaire or can also be asked verbally to know better answers. If questions are asked verbally then its the responsibility of the HR who takes up the Exit Interview to note all answers and document the same.
Usually we do prefer the Questionnaire method as that provides time and privacy for employee to put down the genuine feedback which would be documented with this hand written feedbacks.
If its a system driven then we would ask for the final consent from Employee before he submits what he has entered and would serve a great data point to track and improve on the feedbacks and derive analytics to point out the major issues faced at organization.

3. Did you feel that you were provided with adequate training and resources to do your job effectively?
4. Were you happy with your compensation and benefits package?
5. Did you feel like your ideas and suggestions were valued and considered by your supervisors and colleagues?

7. Was there anything that you found particularly challenging or frustrating about your job or the company as a whole?
8. Did you experience any harassment or discrimination during your time at the company?
9. Would you recommend the company to a friend or colleague? Why or why not?
10. Is there anything else you would like to share about your experience at the company?
